During service, we communicate to your driver what preventative maintenance the fleet vehicle needs based on current
mileage. PKD Holdings LLC/Automobilia Inc. uses vehicle manufacturer and industry expert information to determine which
additional service is necessary to maintain your fleet vehicle’s optimal operating condition. Your driver is under no obligation
to perform these additional services. If you choose not to have additional services performed, it is your responsibility to notify
your driver of your wishes as you will be responsible for any and all services authorized by your driver and performed by PKD
Holdings LLC/Automobilia Inc.. A current price list of services is available at the facility.
Invoices are mailed at month end. Invoice Terms are due upon receipt. To qualify for billed fleets we require a minimum of 3 vehicles
to be serviced per year with a minimum of 5 services total yearly. If payment becomes 30 days past due, the unpaid balance will be
assessed a 1.5% late charge per month and fleet account credit is suspended until full payment is made. Continuous late payment
results in a permanent loss of credit. Any check returned by the bank as unpaid will carry a $25 charge. If the account is listed with a
collection agency or attorney to obtain payment, the applicant is liable for all expenses including attorney’s fees.
The undersigned grants permission to Pkd Holdings LLC/Automobilia Inc.to access the above credit information to establish credit on
this account. If this application is approved and credit extended, the applicant shall be deemed to have agreed to the terms and
conditions listed above.
